chain tension possibly to tight, not lubed properly, or just unfortunate
What happened to you is the classic highside and you are lucky you did not brake a collar bone as most do getting flicked over the bars.

Sorry to hear that. I agree chain being to tight and chain being to loose is a posibility

I would also check your axle alignment. Adjust the chain-side axle jack until you reach the desired chain tension, tighten the axle nut on that side. Now tighten/loosen the other axle jack while you spin the rear wheel and watch the chain riding on the rear sprocket. When the chain rides center on the sprocket, tighten the axle nut. With both axle nuts tight, check the axle jacks to make sure they are snug, but not too tight. Keep in mind that proper chain alignment may cause your bike to crabwalk slightly, or it may fix a crabwalk you already have. Either way, be ready for a change in handling just in case.

You can't rely on distance from the end of the slot or end of swingarm since these things are all made differently, with varying specifications and manufacturing tolerances.

Best tension for the chain: The middle of one side should be able to move up or down at least a half inch, but no more than 1". The chain will not move on the other side. It must not flop about loosely.

i had a similar incident a few days ago, face planted over the bars and spent 3 days in the hospitol after surgery to my elbow "2 pins and wired back together", feel lucky!
